PLEASE READ: choose ONE of the below forms. One form is to unenroll from ALL classes at Balance and one is to drop one or more classes but remain enrolled in at least one class at Balance.
I wish to withdraw my student from ALL classes at Balance.
This form is for students who wish to completely withdraw from the studio. NOTE: If a student is wishing to drop one or more classes but will remain enrolled in one or more classes at Balance, please contact the front desk and someone will assist you.
If you are wishing to completely withdraw from Balance Dance Studios you must complete the form below in order for your dancer to be considered officially unenrolled. Please note that after submitting the form below, you will be sent a confirmation email which is your official withdraw notification. If you do NOT receive the withdraw confirmation email, please contact the Balance front desk ASAP, as the form submission may not have been successful.
I wish to stay enrolled in 1 or more classes but drop 1 or more classes.
This form is for students who wish to withdraw from one or more classes but will remain enrolled at Balance in 1 or more classes.
